I finally tracked down how this long-standing bug is triggered. It can happen when the user pauses and then stops the simulation in quick succession, before the current simulation thread has a chance to terminate (most likely during minimization, which can run to many seconds for a large model). The callback is supposed to remove itself when the thread completes - but in this scenario the thread manager gets deleted first, invalidating its ready-check function. Corrected things to make sure the callback is removed at the moment the simulation is stopped.
