Not sure why "equivalent" commands are being logged here. Maybe somehow related to the earlier "ui view fourup"? (Though that seems kind of unlikely.)

My only guess is this error is caused by the segmentations tool. It looks like a map was added somehow to volume viewer and then closed without volume viewer detecting it was closed. Then the user clicked the "-" icon next to the map in volume viewer to delete it to generate this error.
